CONSUMER DIRECTED SERVICES
The Consumer Directed Services (CDS) Program helps people with significant disabilities obtain assistance with personal care and daily activities to maintain their independence. When services are consumer-directed, the individual and sometimes his or her family decide what service is needed, who will provide it, when it will be provided, where it will be provided, and how it will be provided.

AT-HOME PRIVATE CARE
At Home Private Care Services will build a customized plan of care to meet the client’s specific needs. Services are paid for out of pocket.
Services include but are not limited to the following:
RN’s, LPN’s, CNA’s, Nurse Aides
Rehabilitative Services
(Physical Therapy, Occupational Therapy, and Speech Therapy)
Personal Care Assistance
Companion Services
Errands and Shopping
Transportation Services
